Rooted Community Living

Everyone deserves a place that feels like theirs.

Rooted Community Living is being built around a straightforward idea: that a home should be a home. We are developing home and community-based services for adults in western Iowa — people with intellectual and developmental disabilities, people living with the effects of a brain injury, and people managing long-term mental health conditions. Real households where people live with dignity, independence, comfort, and a genuine say in their own lives, as valued members of their communities.

Planned service area: Pottawattamie County and surrounding communities in western Iowa.

Where we are right now. Rooted Community Living LLC is in the process of enrolling with Iowa Medicaid as a home- and community-based services provider. We are not yet accepting referrals, and no services are being delivered. This page will be updated as our enrollment and certification progress. If you are a case manager, family member, or prospective employee who would like to be notified when we begin accepting referrals, please get in touch.

What we're building

Two services, both delivered where people actually live rather than in a facility.

Supported Community Living

Intellectual Disability and Brain Injury waivers · in development

Daily support in a shared home for a small number of adults — no more than four people in a household. Support is built around each person's own goals and covers what a day actually contains: personal care, cooking and running a household, building skills, getting out into the community, transportation, and having someone alongside you who knows you well. Available under both the Intellectual Disability and Brain Injury waivers, so that a person's diagnosis determines the support they get rather than whether we can serve them at all.

Home-Based Habilitation

Habilitation program · in development

Individualized support that helps people build and keep the skills that make community living work — daily living, personal care, getting out into the community, and the practical business of running a life. Delivered wherever someone lives, including in one of our own households, and at whatever hours of the day or night the support is actually needed.

How we intend to work

These are commitments we are writing into our policies, not slogans.

  1. The home belongs to the people who live there

Your own bedroom, with a door you can lock. Visitors when you want them. Food when you're hungry. A schedule you have a hand in setting. These are rights, not privileges to be earned.

  2. Small on purpose

Households of no more than four. Small enough that staff know each person well, and that daily life can bend around individual routines instead of the other way round.

  3. Support that points outward

A good day includes leaving the house. Work, friendships, appointments you chose, places in the community where people know your name — the point of support is to make more of life possible, not less.

  4. Families and guardians stay in the picture

You should be able to reach us, hear from us before you have to ask, and know what happened on a hard day. We would rather over-communicate.

Who we are

Rooted Community Living LLC is a small, family-run provider in the early stages of development. We are building deliberately: policies and procedures first, then a home, then the right staff — rather than opening quickly and working it out afterwards.

We are an Iowa limited liability company operating from offices in the Omaha–Council Bluffs area, with services planned for western Iowa.
